JUVÉDERM VOLBELLA® XC, The Newest Lip And Under Eye Filler

JUVÉDERM VOLBELLA® XC is the very newest member of the Juvederm family of facial fillers, and can be used to perfect your pout and help erase under eye circles for longer than previously-used fillers. It is a smooth gel formulation made up of a modified form of Hyaluronic Acid, a naturally-occurring sugar found in the human body. The role of Hyaluronic Acid in the skin is to deliver nutrients and help the skin retain its natural moisture and softness. The JUVÉDERM VOLBELLA® XC gel formulation also contains a small quantity of local anesthetic (lidocaine) that helps minimize discomfort during injection.

How Long Does It Last?

The decision to augment the lips is usually one that is easily made by my patients. Either they want fuller lips, or poutier lips, or they don’t. It’s more difficult for people to determine on their own if their dark circles would benefit from JUVÉDERM VOLBELLA® XC injection.

JUVÉDERM VOLBELLA® XC For The Under Eye Area

When it comes to filling under the eyes, the best candidate for injection is someone who’s under eye darkness is the result of “hills and valleys” under the eye. The “valleys” are hollow, sunken spots that, when they’re beside normal, plump areas (“hills”), create shadowing—and the perception of dark circles. To help determine if your circles are shadow-related, sit down in a room with overhead lighting. Take a mirror and first look straight into it while seated and holding the mirror at eye level. Say hello to your circles. Next, lay back a bit, and let the overhead light hit you directly on the face while looking into the mirror, so that all under eye shadows are eliminated. If the darkness under your eyes disappears, it’s shadowing that’s causing the circles, and JUVÉDERM VOLBELLA® XC may be a good option for you. If the darkness remains, the problem is probably excess pigmentation in the skin, something filler will not address.

When The Psychiatrist is the Patient

A Psychiatrist needs to maintain a sympathetic demeanor without displaying a disapproving, “frozen” look. In contrast to the lawyer, this patient needs to seriously weaken movement in the corrugators so they can relax and listen to what their patients are saying without looking judgy, but keep a little movement in the frontalis muscle (the muscle that raises the eyebrows) so they can still look like they care about what they’re hearing.

Let The Teacher Teach

He needs to maintain the ability to look surprised and to be sympathetic. This patient needs to retain almost all movement of the frontalis muscle that raises the eyebrows, but can often benefit greatly from Botox used to soften the crow’s feet and raise up the arch of the eyebrow.

The Poker Player

This patient needs Botox to retain her poker face and help eliminate the look of surprise (frontalis muscle) and some of the most common facial “tells” such as clenching the jaw and flexing the masseter muscle at the angle of the jaw. (masseter muscle Botox) It’s important for poker players to be able to keep all emotion under wraps.

The Actor Needs to Emote

Performers need to be able to express all emotions, but must also look permanently airbrushed with subtle touches of Botox in a number of areas.

Interview With Dermatologist Dr. Mitchell A. Kline

Dr. Mitchell A. Kline is a New York board-certified clinical dermatologist. His ongoing focuses include melanoma, neurophysiology and HIV/AIDS, and he has done significant research on non-invasive melanoma detection techniques, particularly reflectance confocal microscopy, which he uses at his office.

Dr. Kline specializes in melanoma treatment and staging, total body dermoscopy, and DermaSpectra total body digital imaging. While Dr. Kline is considered a go-to doctor in New York City for skin cancer concerns, his office also offers an array of cosmetic treatments including injectables, fillers, photo-facials, and chemical peels. He is one of the few doctors in New York to offer PRP facials (Vampire Facials), as well as PRP for hair regeneration.

Dr. Kline studied at the Perelman School of Medicine at the University of Pennsylvania, training in Internal Medicine at the University of Pennsylvania Graduate Hospital following his graduation. He participated in research at the Children’s Hospital of Philadelphia in Pediatric Neurosurgery and Neurophysiology, receiving a Young Investigator Award from the National Institutes of Health. Subsequent training in Dermatology was completed at Cornell, Rockefeller University and Memorial Sloan-Kettering. He was Chief Resident in Dermatology at Memorial Sloan-Kettering/Cornell University from 1989-1990. He is also on the Clinical Board of Melanoma Research Alliance and honorary of the year by Melanoma Research Foundation.

His work and research have been the subject of stories on CBS, The Wall Street Journal, The New Yorker, and featured on the HBO documentary “Cancer: Urgency to Emergency.” We recently had the pleasure of speaking with Dr. Kline about his work.

You Are Well-known For Your Expertise In The Field of Skin Cancer. Was Skin Cancer Detection and Treatment Your Original Focus?

MK: Paths are complicated. Perhaps the best way to describe it is that melanoma detection requires a melding of mathematics, physics of optics and light, an understanding of visual perception, as in art, and an appreciation for humanistic parts of caring for people.

What Is Something You Wish Every Woman Knew About Melanoma and Melanoma Prevention?

MK: That putting sunscreen on face, ears, scalp and hands as a daily habit after showering will make an enormous difference in skin quality and cancer risk. The habit will continue for beach and the great exposures of daily use.

You Are One of the Few Physicians in New York to Offer the “Vampire Facial”. What Happens During this Treatment and What Are Results?

MK: The use of Platelet Rich Plasma Therapy or PRP, on both hair growth and the effects on facial collagen are well known as variable but often superb for decades to orthopedists who treat athletes and injuries. The vampire facial utilizes the same simple technique whereby platelet-rich plasma is separated from the other components of the patient’s blood and then injected into the skin or scalp. Over 5 successive treatments and in combination with topical agents of proven value can result in collagen and hyaluronidase acid stimulation, texture improvement and reduction of wrinkles. There is typically no or little down time and minimal discomfort.

If Women Want to Start Preventative Cosmetic Treatments in Their Early 30s, or Earlier, What Are the Best Treatments to Start With?

MK: Of course, moisturize photoprotectants and retinol, retinoids and antioxidants dramatically prevent signs of aging and damage that causes skin cancer.

Depending upon the ways in which they use their muscles of facial expression to form creases, have genetic propensity for aging, brown spots or wrinkles, we would tailor such treatments. Treatments can range from specific topical antioxidant creams and gels scientifically backed with known benefits over years, light IPL (Intense Pulsed Light) to remove veins, red areas and brown spots, to delicate Botox and fillers that block progression of future lines without creating any change at in the beautiful natural contours and shapes of the face.

Underappreciated is the benefit of rest, diet and supplements that truly affect the way the skin ages and damages. The science is astounding and soon will become part of prevention rather than treating problems that develop.

Any New and Exciting Developments and Products on the Horizon for 2016-2017?

MK: A giant step is the development of a cream that will dissolve brown spots known as keratoses. It is the most common thing we see on people’s skins at every age especially later.

Huge changes are at hand with radio frequency tools, topical anti-aging agents, some that will not require any needles or invasive treatments.

Anti-Aging Treatment Recommendations from M.B. York

Having been an aesthetician and spa owner, I am well-versed in various anti-aging treatments like microdermabrasion, photo facials, etc. I have achieved great results for both myself and my clients with these treatments, but they can only take you so far. As a makeup artist, I always teach my clients tricks for smoother-looking skin: The right moisturizers, exfoliators, primers and foundations. Still, some clients remain unhappy about the signs of aging, such as sagging jowls and neck, age spots, wrinkles, enlarged pores and uneven coloration…all of which need more help than superficial concealing makeup can provide.

I have had the unique opportunity to observe a wide array of anti-aging treatment modalities offered by the many medi-spas that carry my M.B. YORK products. Even with my extensive knowledge, I am often confused by all this technology, and secretly wish someone could offer a simple explanation of what devices and treatments could achieve profound anti-aging results. As much time as I have spent in these environments, I remain frustrated, still not clear about exactly what to do to receive the results my clients and I am looking for.

One of my makeup clients has flawless skin. When I asked her secret, she told me about Robert Mazurek, owner of Rx Skin Clinic, so I decided to check it out. When I went to his office, he examined my skin with a Wood’s lamp, which reveals the extent of sun damage. He provided a full consultation, explaining how my main concerns with aging (reversing sagging, sun damage, and wrinkles) all need to be treated in different ways. He advised photofacials for my sun damage, skin tightening laser for the sagging, and Pixel laser for smoothing wrinkles and enlarged pores. He noticed the spots on my hands and offered to include two photo facial hand treatments to be added at no charge if I were to purchase the package of 6 face & neck treatments sessions. As a makeup artist, I’ve long been self-concious of my sun damaged hands, since they are always front and center, often closeup on camera to capture my makeup technique.

While I was nervous about how much this would cost, I was also excited as I looked over his compelling before and after photos. He informed me that he has performed over 30,000 laser treatments, and the way he expertly explained everything made me trust him immediately.

My first treatment consisted of a photofacial and skin tightening laser treatment for face & neck. I was worried about the pain as he didn’t numb my skin first— but it wasn’t bad. It felt like the snap of a rubber band on the skin. For sensitive clients, topical numbing cream is available. The treatments took about 20 minutes and there was zero downtime! My skin looked a little red— which was gone in an hour—yet vibrant and stimulated. I couldn’t wait for my second treatment.

A month later I went in for my second skin tightening treatment, followed by my first Pixel laser treatment. The latter required some topical numbing cream to be applied afterward to soothe the sunburn-like sensation. I felt some heat, but no pain at all. Once again, the two treatments were completed in about 20 minutes. My face looked more red than the first treatment, so I was given some topical healing ointment to apply every 4 hours for the next day. I couldn’t wear makeup for 24 hours, since all of the pores were temporarily exposed from the penetration of the laser. Though I felt self conscious, no one could tell a thing unless I pointed out how my skin was redder than usual. My skin also felt like sandpaper as it developed microscopic rough dots where the laser made “Pixel” perforations. You really couldn’t see them, though—I only felt them if I touched my face. After a week, all this “rough” skin fell away and underneath was brand new baby skin. My moisturizers worked better, delivering deeper hydration than before, and my fine lines were disappearing! I also had my first hand photofacial, the results of which Robert said are the equivalent of 2 face photo facials. Any freckles or sunspots became temporarily darker, but not scabby or different in texture. He explained that it would take longer than the face (almost three weeks rather than one) for the spots to completely fall away. I am about two thirds of the way there and my hands are looking at least 10 years younger!

Robert explained that this series of treatments can potentially take 20 years off your skin, and I am already seeing huge progress! I’m excited to see improvements with each session, and of course the final results after finishing the series. And it doesn’t stop there: The improvements continue for five months after the final session as new collagen continues to regenerate and grow. It is like real life photoshopping!

Facial Plastic Surgery Trends and Predictions for 2015

Believe it or not, facial plastic surgery, like fashion, is subject to trends. What’s “hot” changes constantly, and who wants which procedure changes as well. Over the past few months, it seems that demand for liquid surgery (without the knife) has increased, and men are beginning to turn up for tweaking more frequently. There are also some exciting new products waiting for the FDA nod in 2015. Here’s what I have on my radar for the year to come.

Liquid Courage

Basically, anything non-surgical, or “liquid” is exploding in popularity in my office. The two most common, non-surgical “liquid” procedures I’ve seen a major increase in are Liquid Rhinoplasty and the Liquid Facelift. At this time last year, I was performing about 1 Liquid Rhinoplasty per month. Now, it’s more like 1 or 2 per week!

In these “liquid” non-surgical procedures, a combination of Botox and various facial fillers are injected to achieve the desired result without a scalpel. In the right candidate, a Liquid Rhinoplasty can even out irregularities along the bridge of the nose, raise the nasal bridge, or camouflage a dorsal hump on the profile view. Similarly, in a Liquid Facelift, a combination of Botox and filler is used to turn back time, rejuvenate the area around the eyes and mouth, erase fine lines and wrinkles, and replace the volume that our faces tend to lose with time. During a liquid procedure, ice is used to numb the skin, and small needles are used to inject filler and/or Botox. Not all nose concerns are amenable to Liquid, non-surgical, Rhinoplasty or Facelift, but if you are among the lucky (or unlucky, depending upon how you look at it), you can skip the pain and recovery of surgery. Results are dramatic and instant, and so is the savings!

Menaissance

Over the past few months, I’ve seen a serious increase in the number of men coming in for cosmetic pick-me-ups, and I expect the trend to continue in 2015. To what do we owe this “BRO-tox Boom?” When I ask, the main reason men report seeking out cosmetic surgery treatments is to appear younger at work. It seems that the combination of increased availability of easy outpatient treatments, wider cultural acceptance of cosmetic procedures, and an increased emphasis on the value of youth in the workforce has fueled a “menaissance” in facial plastic surgery.

The American Society of Plastic Surgeons has reported that the number of men seeking Botox has increased 310 percent since 2000, and I’ve seen a similar increase with many other facial plastic surgery procedures, including facial filler injection, laser treatments, rhinoplasty, and neck liposuction. I would guestimate that three years ago, less than 10% of my patients were men. Presently, I would say that around 30% are.

On the Horizon

Get ready for this: A needleless Botox may receive FDA approval in 2015. There is a new, investigational, formulation of botulinum toxin type A that uses a proprietary peptide technology for delivery across the skin. It’s not your mother’s wrinkle cream, it’s a Botox cream!  The topical treatment will still need to be applied by medical personnel in a doctor’s office, so it’s not a do-it-yourself home treatment. However, it may be useful for people who are afraid of needles, or for large area Botox treatments, such as treatments for excessive underarm perspiration (Yes, you can do that!).

Another cosmetic advancement on the horizon: New fat-melting injections await FDA approval in 2015. Currently in clinical trials, this treatment shrinks fat cells with targeted injections. Once injected, the drug activates receptors within fat cells and triggers the breakdown of triglycerides, causing the cells to lose volume. Translation? Bye bye flab, hello fab. Stay tuned!
